How to register
- visit reception, Monday to Friday, and ask for a registration form
Please bring photo ID. It helps to have your NHS number – find your NHS number (nhs.uk).
If you want to use online services (booking, prescriptions and your record), you must bring photo ID so we can confirm who you are.
